Diagnostic and screening procedures

Specific diagnostic procedures e.g. phlebotomy, microbiology samples are undertaken for analysis off-site. Specific screening programs such as cervical screening are also undertaken for analysis off-site. Other procedures such as electrocardiograms and spirometry are undertaken on-site with results available on-site.

Maternity and midwifery services

Services are offered and provided in conjunction with community midwives for the assessment, treatment and education of patients in the antenatal period as well as post-delivery.

Treatment of disease, disorder or injury

General practice services for our registered patients and on occasions, patients registered with other GP practices or temporary residents.

Family planning services

Provision of all general family planning advice and prescription of oral contraceptive, fitting and removal of IUCD devices and contraceptive implants.

Alcohol Dependency

If you have been invited to submit an alcohol consumption review, please complete our online Alcohol Consumption Review form.

Blood Pressure

If you have been advised by the surgery to submit your blood pressure readings on a regular basis, please complete our online Blood Pressure Review form.

Cervical Cytology (Smear Tests)

All women between the age of 25 and 49 years should have a test every three years; and between 50 – 64 years, every five years, except if they have had a hysterectomy. Guidance on breast examination will also be offered in line with current recommendations. Please make an appointment with the Practice nurse if you are called by the Primary Care Support Service, or if you may be overdue. It is important to keep your smears up-to-date.

Child Health and Immunisations

Please make an appointment with the Practice nurse for immunisations. Further information can be found on our Vaccinations Page.

Chlamydia Test (Free)

If you are between the ages of 15- 25 you can have a free Chlamydia test. This is done on a urine sample. The results are confidential and can be sent to your mobile phone. Ask the doctors, advanced nurse practitioners or nurses for the test visit www.nhs.uk.

Dietetics

Please make an appointment with the Doctor or Practice Nurse. A referral to the dietician may be made where appropriate.

Drug Abuse

Please make an appointment with the Doctor.

Ear Syringing

Please make an appointment with the Practice nurse, and use wax softening drops (available from the pharmacy) or olive oil before attending.

Family Planning and Sexual Health

If you have been advised by the surgery to submit a contraceptive pill review please use this form.

Please make an appointment with the Practice nurse for contraceptive advice. Coil fitting / removal, and diaphragm fitting is carried out by appointment.

Flu Vaccinations

Flu vaccinations are carried out each year for certain at risk groups.

Our flu champions are Salilamol Pillai (Lead Nurse) and Charlene Beck (Health Care Assistant) with weekend drop in clinics planned in September. Patients are encouraged to book in for flu clinic appointments from August.

NHS Health Checks

Our Practice is pleased to be offering a free NHS Health Check to all our registered patients between the ages of 40 and 74 years who do not have an existing diagnosis of diabetes, heart disease or stroke. (These patients will already be receiving care and attention from the practice).

The latest guidance we have is that we no longer need to perform Health Checks on patients under 40 years – although we can do this where there is a particular wish on the part of the patient.

The NHS Health Check is a fairly short but comprehensive assessment with a trained Health Care Assistant and will include Blood Pressure, weight, diet advice, smoking advice and cholesterol measurements amongst other things.

We strongly recommend all our target population attending the surgery for these checks.

Simply speak to reception and book yourself in, if you are aged 40-74 years.

Over 75 Years

You are invited for a yearly check-up with the Practice nurse or the Doctor. This is entirely optional. You may request it to be at your home if you are housebound.

Shingles Vaccines

Shingles vaccines given at 70 and 79 for all patients.

Teenage Health

Please make an appointment with the Practice nurse. The Doctors and nurses respect the confidentiality of patients under 16 who request an appointment without an accompanying adult. However, it is still strongly recommended that under 16s should come with a parent or guardian.

Private Non-NHS Work:

Medical Examinations (Non-NHS)

Certain services provided by the Doctors are not covered by the NHS and fees are payable for such items as:

  • HGV & PSV licences
  • Elderly driver licences
  • Insurance
  • Pre-employment medicals
  • Private certificates
  • Holiday vaccinations
  • Insurance claims
  • Fitness for sports

Please ask the receptionist for such services. Charges are payable at the time of attendance, and the BMA recommended scale of fees can be found on our Non-NHS Work page, or you can go into the practice and ask a Receptionist.

Medical Certificates

If you have been ill for more than 7 days you will need a doctor’s certificate. These certificates are called ‘Fitness To Work’ Certificates. The doctor may call you or arrange for an appointment.

If you have been ill for 7 days or less you don’t need to see a Doctor. You can complete a Self Certification form yourself. However some employers insist on a Doctor’s note regardless. A Doctor’s note under these circumstances may involve a charge.

Many employers have their own self-certification forms. If your employer doesn’t have its own form you can download the Self Certification Form. Please print it, fill it in and hand it in to your employer. You do not need to see a Doctor.

Private Patients

Patients, who are not NHS registered at this surgery, can be seen privately. Please contact the Practice Manager regarding fees.

Fees

The current commercial fee will be charged. A cash deposit will be required when booking your appointment, which is non-returnable if you do not attend your appointment.

  • Medicals (Taxi, Bus or HGV) £175 (deposit of £50 and £125 payable on day of medical)
  • GP Report (Solicitor or employer) £117
  • COP3 form £160

Smoking Cessation

If you have been advised by the surgery to submit a smoking review on a regular basis please use this form.

Tetanus and Polio

If you are travelling abroad please make sure you contact us in plenty of time to arrange any vaccinations that may be necessary. To help the Travel Nurses assess your travel needs it is important that they are in receipt of the assessment form before your appointment.

Adults should have a full course if not previously immunised, otherwise a booster every five to ten years (tetanus), or for foreign travel, every ten years (polio). Please make an appointment with the Practice nurse.

Well Person
